OCTOPUS · OCTOPUS tested metformin and alpha-lipoic acid in progressive MS. After an interim review, the alpha-lipoic acid arm continues to Stage 2 and the metformin arm stops.
What is the approach?
OCTOPUS is an adaptive trial platform in progressive MS: it can test several treatment groups against a shared comparison group and change groups as evidence accumulates. Its initial medicines include metformin and alpha-lipoic acid, chosen for possible neuroprotective or repair-related effects; those effects in patients are questions the platform tests.

Research stage
Phase 3 study evidence; current human-study status is unknown
The programme is listed as active; current human-study status is unknown.
What we know so far
The trial team reported an interim MRI and clinical signal for alpha-lipoic acid on 17 September 2026. This supports further testing, not a final result on benefit or safety. The metformin arm did not advance.
What this means for patients today
This is a UK and Australian research study, not an available treatment. UK residents could register interest in Stage 2 at the 17 September announcement; current enrolment and eligibility require direct confirmation. No Greek centre is confirmed. The interim decision does not establish clinical benefit.
Treatment or approach
Alpha-lipoic acid; metformin arm stopped
How it is intended to work
Reuses existing drugs to support cell energy and reduce damaging oxidation
Where the study is running
UK trial with Australian extension; UK Stage 2 interest invited on 17 September 2026; no Greek site confirmed
The first OCTOPUS review in progressive MS will assess metformin and alpha-lipoic acid. It will look at brain shrinkage on MRI, disability, walking speed and hand function.
